BBL recovery is different from any other plastic surgery. The primary restriction isn't pain or swelling — both are managed with medication — it's body position. For 6 to 8 weeks after surgery, you cannot sit directly on your buttocks. This is the non-negotiable constraint that patients must plan for before booking their procedure.
The reason is biological, not arbitrary. The transferred fat cells spend the first 6–8 weeks establishing their blood supply in the gluteal tissue. Direct pressure on the grafted area during this vascularization window compresses newly transplanted cells, reduces survival, and diminishes the final result. The BBL pillow — a donut-shaped cushion that redirects weight to the thighs — makes this restriction manageable, but patients who cannot commit to this protocol consistently should wait until they can.

Sleeping face-down or on your side is recommended for the first weeks. When seated, the BBL pillow must be used on every surface — chair, car seat, airplane seat, toilet. International patients flying home after their Bogotá recovery period need to use the pillow for the flight. Most airlines do not have restrictions on BBL pillows in cabin baggage.
Planning your work setup before surgery makes a significant difference: a standing desk setup or the ability to work from a reclined position for the first 2–3 weeks dramatically reduces compliance difficulty.
The compression garment covers the liposuction donor zones — not the buttocks. It is worn continuously for the first 4–6 weeks, removed only for bathing. It reduces swelling in the donor areas and improves contour definition by preventing fluid accumulation and supporting skin retraction.

Contact your surgeon immediately if you have: fever above 38°C (100.4°F) that does not resolve, chest pain or difficulty breathing, localized redness or swelling that is progressively increasing, foul-smelling discharge from wounds, or bluish discoloration of the gluteal skin.
Fat embolism and deep vein thrombosis are rare but serious complications. Symptoms include sudden difficulty breathing, leg pain with swelling, or chest tightness.
