Breast explantation is the surgery indicated to remove breast implants, whether due to a medical complication, implant deterioration, or personal decision. It is a procedure that requires the same rigorous surgical planning as the original placement: the chosen technique —simple, partial capsulectomy, or en bloc— completely changes the result, the scar, and the need for additional surgeries.

Preoperative evaluation determines the technique and the need for additional surgery
Explantation is indicated when there is grade III or IV capsular contracture (hard and distorted breast), implant rupture confirmed by ultrasound or MRI, chronic pain without an identifiable cause, or diagnosis of ASIA syndrome. It is also indicated in the detection of BIA-ALCL associated with textured implants, although it is an extremely rare complication.
Many patients choose explantation without a medical cause: they change their aesthetic preferences, do not wish to maintain implants long-term, or look for a more natural appearance.
