The four rosacea subtypes and how we address them
Not all rosacea is the same. The subtype determines which technology and protocol give the best result for your particular case.
Erythematotelangiectatic
Persistent central redness + couperose (visible vessels). The most common subtype. Responds very well to IPL and Nd:YAG laser for the vessels.
Papulopustular
Redness + papules and pustules without comedones. Requires topical or systemic medical treatment combined with IPL for vascular control.
Phymatous (rhinophyma)
Progressive thickening of nasal skin. More common in men. Treated with ablative CO2 or Er:YAG laser for tissue remodeling.
Rosacea treatment protocol at ALMO Clinic

1. Dermoscope evaluation
We classify the subtype, assess skin phototype (phototypes III–V are common among Colombian patients), and rule out coexisting conditions such as seborrheic dermatitis or lupus erythematosus.
2. IPL or Nd:YAG laser for vessels and redness
Intense pulsed light (IPL) targets diffuse redness and superficial vessels. The Nd:YAG 1064nm laser is the option for deeper vessels and darker skin, where IPL carries a higher risk of hyperpigmentation.
3. Topical maintenance treatment
Topical ivermectin or metronidazole between sessions to maintain inflammatory control. Barrier care routine with soothing ingredients (allantoin, niacinamide) and mandatory mineral SPF50+.
