Sculptra is the brand name for an injectable collagen biostimulator whose active ingredient is poly-L-lactic acid (PLLA), a synthetic, biocompatible, biodegradable polymer that has been used in medicine for over two decades — including in absorbable surgical sutures — before being applied in facial aesthetics.

The product comes as PLLA microspheres suspended in a sterile water vehicle with carboxymethylcellulose. Once reconstituted, it’s injected into the deep dermis or subcutaneous plane, depending on the area being treated.

It’s important to understand this from the start: Sculptra is not a dermal filler. It doesn’t physically occupy space the way hyaluronic acid does. Its function is entirely different — and that’s exactly what makes it so different in terms of results and expectations.

How does Sculptra work in the skin?

PLLA doesn’t fill — it triggers a biological response. When injected into the dermis, the microspheres cause a controlled inflammatory reaction that draws fibroblasts — the cells responsible for producing collagen — to the treated area.

Over the following weeks, those fibroblasts begin depositing new type I collagen around the microspheres. This process is slow by design: the PLLA gradually breaks down in the body (metabolizing into water and carbon dioxide), while new collagen naturally takes its place.

That’s why Sculptra’s results don’t look like those of a filler: there’s no single-day “before and after.” It’s a process that builds session after session, and that keeps improving even after the full protocol is complete, as the new collagen matures.

Which facial areas is Sculptra applied to?

One area where Sculptra is contraindicated: the eye contour (eyelids, crow’s feet). Periocular skin is too thin for PLLA, and applying it there increases the risk of palpable or visible nodules.   2. Preparation: the product is reconstituted with local anesthetic, making the application more comfortable.
  3. Application: injected with a needle or cannula depending on the area, generally into the deep subcutaneous plane.
